Scope of Work: (Contractor must obtain and submit all permits prior to performing any work.) The Project consists of the demolition of aluminum ceiling slats and lighting, and installation of new LED light fixtures at the Tri-Rail Station's second floor bridge. (REFER TO SUMMARY OF WORK) In accordance with Miami-Dade County Implementing Order 3-9, Accounts Receivable Adjustments, if money is owed by the Contractor to the County, whether under this Contract or for any other purpose, the County reserves the right to retain such amount from payment due by County to the Contractor under this Contract. Such retained amount shall be applied to the amount owed by the Contractor to the County. The Contractor shall have no further claim to such retained amounts which shall be deemed full accord and satisfaction of the amount due by the County to the Contractor for the applicable payment due herein. 1. Sealed Envelopes (only) with the Department Name project name, project Number and the Number of the Request for Price Quotation (RPQ) noted on the outside of the envelope 2. Number of Copies of Bids: Three (3) one (1) original and two (2) copies 3. Bid Bond Required: YES totaling 5% of bid submittal price 4. Where to Submit Bids: All bid submittals must be sent to the attention of: Office of the Clerk of the Board 111 NW 1st Street, 17th Floor , Miami, FL 33128, 2:00 p.m. NOTE: Late Bids submittals will not be opened. 5. Submit RFI's to email@example.com and copy firstname.lastname@example.org - DEADLINE FOR RECEIPT OF RFI QUESTIONS: COB 12/5/2018. 6. This Request for Price Quotation (RPQ) is for a Miscellaneous Construction Contract. 7. In the event the lowest responsible and responsive bid amount exceeds the project's cost estimate, Miami Dade County may hold a meeting to request further cost reductions in an effort to align the bid amount with the project's cost estimate. However, Miami Dade County will not engage in any type of negotiations or modifications of the original scope, terms or conditions other than the price reduction. 8. All terms and conditions of the MCC 7360 are part of this contract and will be enforced. 9. Refer to Article 2.14 below of the MCC 7360 to further clarify the license requirements: 2.14 LICENSE QUALIFICATIONS OF CONTACTORS: A. All Contractors must hold a current valid Certificate of Competency for General Building/Engineering or Specialty Trades Contracting, as required by the Florida Building Code, for the types of Work covered by the Contract at the time of RPQ submission and maintain same throughout the duration of the project. The certificate(s) is to be issued by: 1. The State of Florida Construction Industry Licensing Board, pursuant to the provisions of Section 489.115 of the Florida Statute and registered with the Miami-Dade County, Building Department or, 2. The Dade County Construction Trades Qualifying Board, pursuant to the provisions of Section 10-3(a) of the County Code. Holders of Miami-Dade County Certificates of Competency must also hold Certificates of Registration issued by the State of Florida Construction Licensing Board, pursuant to the provisions of Section 489.115 or Section 489.117 of the Florida Statutes. B. Proof of such Certificate(s) must be submitted at the time of initial response and maintained current throughout the contract period. The County may request proof of continued certification at any time during the contract period. Failure to provide such proof within five (5) working days from notification by the County shall result in the removal from the contract and the rejection of any current or future RPQ bid submissions. C. Subsequent to the commencement of the Contract, the County may require specific qualifications based on a Project’s scope of work. Such requirements will be included within the RPQ. 10. PRIME CONTRACTOR MUST BE ABLE TO PULL MASTER PERMIT. 11. A 10% Retainage will be held with each payment. 12. Unless specified in the bid form, applicable permit fees must be paid by selected contractor. 13. DBE Goal Recommendation: Race-neutral 14. Davis Bacon Wages (Refer to contract documents - Building and Highway).
